BACKROUND CHECK

Background checks will be completed for all participants over the age of 18.

PASSPORT COPY

Upload a copy of your passport so we can book your airline tickets.

PHYSICIAN RELEASE FOR MINOR

Any participant under 18 needs to have approval from a physician.

MEDICAL INFO FORM

All participants will complete a medical information form to help your medical mentor keep you safe and healthy during the international trip.
