Applicants must be a Junior or Senior in a Utah high school or must be enrolled in a full-time undergraduate college or university program in the state of Utah. Graduate students are not eligible for this award.

Please submit transcripts from your educational institution as proof of your enrollment at a Utah school. Transcripts are requested only as proof of enrollment. The Award recognizes service, not GPA. Unofficial transcripts will be accepted.

To complete your application, we must receive two recommendation forms from people who are very familiar with your service.
